Potential Inc. is now a network provider for: AmeriHealth, Keystone Health Plan East, Independence Blue Cross, QCC, Aetna, and United Behavioral Health.

Medical Assistance will not pay for Potential Inc. services for anyone under the age of 21 at this time. Medical assistance includes plans such as Keystone Mercy and Health Partners.

If you have one of these insurance companies you should fill out the Private Insurance Coverage for Children with Disorders Coverage Certification form from the PA Department of Public Welfare. A copy of the form can be attained by clicking here or by visiting www.dpw.state.pa.us/servicesprograms/autism/act62/especiallyproviders/

If you feel that you are covered after filling out the PA Department of Public Welfare form please send us an intake form, reinforcement assessment and a copy of your insurance information. We will then contact your insurance company about coverage. If you have any questions, please contact our office at 888-AUTISM-0.
